New Hires



Welcome to Westport Community Schools. As a new hire, there are several documents you’ll need to complete prior to being fully on boarded as a employee. This packet is available from the Human Resources Office. She will call you for an appointment after your interview with the Superintendent of Schools.

These documents contained in your New Hire folder must be submitted NO LATER than the day prior to starting at Westport Community Schools. As a reminder, you will not receive a letter of appointment until these documents have been submitted.

*Please DO NOT print these documents unless an error is made, for most accurate and up to date please use the documents in your packet!*

To be submitted to Human Resources:

  1. W4 – Employee’s Withholding Allowance Certificate
  2. Payroll Check Mailing Process Form
  3. Direct Deposit form (Mandatory District Wide as of September 1, 2015) and attach voided check.
  4. Form SSA-1945 – Statement Concerning Your Employment in a Job Not Covered by Social Security
  5. I-9 Employment Eligibility Verification form – Complete section 1. Please note that you must bring back with you either 1 document from List A OR 1 document from List B AND 1 from List C
  6. CORI check authorization form. Please note that you must bring a valid state approved form of identification to be copied.

Please submit your official college transcripts (remember to include all credits for proper lane credit on the WFT Collective Bargaining Agreement Salary Scale) and your MA DESE license if applicable for your position. Specialists (Nurses, Occupational Therapists, Speech Language Pathologists, etc.) please submit a copy of your license (Allied Health or R.N.).

Benefits Information to be submitted to the Town of Westport Treasurer’s Office. This packet must be completed no later than your FIFTH day of employment. Please visit the Treasurer REGARDLESS of whether or not you are accepting benefits.

To be submitted to Westport Treasurer’s Office:

  1. Bristol County Retirement forms along with a copy of your birth certificate. IF YOU ARE ON MASS. TEACHERS RETIREMENT, THIS DOES NOT APPLY TO YOU.
  2. S.S.A. Form 1945 (Statement Concerning Your Employment)
  3. HIRD (Employee Health Insurance Responsibility Disclosure Form) PT employees – fill out only if you decline tax exempt healthcare premiums deducted through payroll.
  4. Heath Insurance Enrollment Application
  5. Life Insurance Enrollment Card or signed Declination Card
  6. Volunary Life Insurance Card or signed Declination Card
  7. Signed Flexible Benefits Plan Form
  8. Dental Insurance Application
  9. Short Term Disability Application

Any questions regarding the above please call the Treasurer’s Office at (508) 636-1007. As a reminder, if you elect not to obtain any benefits you may be required to wait until open enrollment or a qualifying event (marriage, birth, death, divorce) to add.

If you have already been fingerprinted for another DESE organization and you meet the information below, please request to have your suitability determination sent directly from your previous employer to the Westport Community Schools Human Resources & Data Administrator.

When an individual for whom a school employer or the Department of Elementary and Secondary Education has made a suitability determination applies to work for a school employer, the school employer may obtain and may rely on a favorable suitability determination, if the following criteria are met:

  1. The suitability determination was made within the last seven years; and
  2. The individual has not resided outside Massachusetts for any period longer than three years since the suitability determination was made; and either
  3. The individual has been employed continuously for one or more school employers or has gaps totaling no more than two years in his or her employment for school employers; or
  4. If the individual works as a substitute employee, the individual is still deemed suitable for employment by the school employer who made a favorable suitability determination. Upon request of another school employer, the initial school employer shall provide documentation that the individual is still deemed suitable for employment by the initial school employer.
