Reporting Incidents of Bullying

Westport Community School’s Concerns Reporting Form is NOT an emergency hotline. If you are reporting an emergency that demands immediate response for safety reasons, please call 911.

All students deserve a safe and supportive learning environment free from bullying, harassment, bias, and other obstacles to education. Westport Community Schools are committed to promptly investigating all reported concerns, and to taking prompt action. This WCS Concerns Reporting Form is intended to facilitate the reporting of sensitive information to school and central office officials. 

This form is for sharing concerns related to the school district, including but not limited to information regarding reports of potential bullying, cyberbullying, student safety, substance use, harassment, bias-based incidents, violence, retaliation for reporting, and other misconduct, as well as tips about students who may be struggling or in need of assistance.

If you would like us to conduct an investigation, or otherwise follow up with you regarding your report, please be sure to provide your name and contact information. We do not collect or store any personal information about you when you submit your alert tip unless you voluntarily provide it. 

The forms below can be used to report incidents of bullying that involve Westport Community School students and staff.  For convenience, the form is available in both Microsoft Word format and as a PDF.  Please complete the form to the best of your ability and submit it to the main office in any of the schools (via email, postal service, or drop-off).
