Athletic User Fee Waiver Form

All student-athletes must register for athletics via FamilyID. Click the Register Now FamilyID button above to register. A copy of a valid physical exam (within 13 months) must be submitted in order to participate.

Registration numbers will determine which high school & middle school teams we will be able to offer each season. Please understand that decisions will be made in order to keep as many kids participating as possible.
